•Most research does not examine how musculoskeletal adaptations to BFRT transfer to aspects emphasized in the rehabilitation field, such as the individual’s physical functionality and their physical fitness.•There seems to exist a low rate of outcomes associated with physical function published in literature assessing the effectiveness of BFRT on ACL rehabilitation.•There is great heterogeneity in BFRT prescription and a greater tendency for studies to only assess two dimensions of physical fitness (strength and muscle mass), which reveals a trend towards a narrow perspective on the role of BFRT in ACL rehabilitation.•Future investigations using standardized outcomes, more homogeneous parameters and specific protocols are needed to better compare patients’ physical function, so that BFRT can be used more effectively in clinical practice.
