Abstract
Background: Children living in poverty disproportionately experience suboptimal or negative social determinants of health (SDH), which can result in poor health outcomes. The City of Milwaukee, Wisconsin has high child poverty rates and child health disparities for Black and Hispanic children. While many sectors support families’ in accessing resources to address SDH, efforts often occur in silos resulting in fragmented, redundant, and suboptimal care. Community-wide collaboration is needed to support family/child resiliency and advance towards child health equity.
