Postoperative Analgesia of Parecoxib in Elderly Patients Receiving Total Knee Replacement and its Effect on Immune Function and Inflammatory Response

The present study aimed to investigate the postoperative analgesic effect of parecoxib combined with femoral nerve block in elderly patients receiving total knee replacement and its effect on immune function and inflammatory response. Seventy patients with knee osteoarthritis receiving unilateral total knee replacement were randomly divided into control and observation groups, 35 cases in each group. After surgery, the control group was treated with femoral nerve block, while the observation group was treated with parecoxib combined with femoral nerve block. The analgesic effect, knee joint function, immune function and inflammatory response of patients were evaluated. After surgery, compared with control group, in observation group the Visual Analogue Scale score was significantly decreased (P < 0.05), the hospital for special surgery knee score was significantly increased (P < 0.05), the plasma CD4(+)/CD8(+) ratio was significantly increased (P < 0.05), and the serum tumor necrosis factor a, interleukin 6 and C-reactive protein levels were significantly decreased (P < 0.05). In conclusion, in elderly patients receiving total knee replacement, the application of parecoxib combined with femoral nerve block can reduce the postoperative pain, improve the immune function and decrease the inflammatory response, thus promoting the recovery of knee joint function.
