Functional hard and soft tissue reconstruction by immediate provisionalization of Osseo Speed Profile implants in the esthetic zone: 3-year results

Aim: The aim of this bi-center prospective cohort study was to evaluate implant survival, marginal bone maintenance, and soft tissue esthetics of immediately provisionalized Osseo Speed Profile (TM) implants (Dentsply Implants) in the esthetic zone within a follow-up period of 3 years. Material and Methods: A total of 29 implants were predominantly placed immediately following tooth extraction (n 25) in 21 patients. The facial ridge defect was grafted simultaneously with autogenous bone chips from the mandibular ramus, and the implants were provisionalized immediately. At 3 months, the final restoration was delivered. Patient follow-up continued for 3 years. Implant success, soft tissue esthetics, and marginal bone levels were assessed. Results: One of the 29 implants that was placed was lost. The other 28 implants osseointegrated uneventfully. One patient did not return for follow-up assessment. The mean follow-up period was 43.5 months. The survival rate was 96.4%. The mean pink esthetic score (PES) ratings improved from 11.4 to 11.9 during the treatment and follow-up. In 74% of the implants, the PES was preserved or even improved. The mean interproximal marginal bone level at the final examination was -0.18 mm. Conclusion: Preserved marginal bone levels and high esthetic results suggest proof of principle for immediate provisionalization of OsseoSpeed Profile implants for immediate as well as staged implant placement in the esthetic zone in situations where a height difference presents in the orofacial alveolar crest.
