Twin Pregnancy with Coexistence of a Healthy Fetus and a Complete Hydatidiform Mole A Case Report

BACKGROUND: The occurrence of a twin pregnancy with a viable fetus and a complete hydatidiform mole is a rare event and bears the risk of several potentially serious fetal and/or maternal complications. CASE: We report an unusual case of a twin pregnancy with a viable fetus and a complete hydatidiform mole that proceeded without complications and resulted in term birth of a healthy infant. The reported pregnancy occurred after natural conception without medical assistance. A cesarean section was performed in the 39th week of gestation and resulted in the delivery of a healthy female neonate and a normal-appearing placenta at the posterior uterine wall. Furthermore, at the anterior uterine wall, without connection to the previously mentioned placenta, a vesicular mass was found and curetted. Pathological examination of the latter revealed a complete hydatidiform mole. The infant and mother did well, with decreasing serial maternal serum beta-hCG levels and no evidence of a persistent gestational trophoblastic disease in 2 years of follow-up. CONCLUSION: The present report shows that, rarely, twin pregnancy with a viable fetus and a complete hydatidiform mole can occur with an uncomplicated pregnancy as well as term birth and without subsequent complications for mother and infant.
