Abstract
In May 2014, 13 people were injured in a fire at a museum that resulted from a chemistry demonstration called the Flaming Tornado. The U.S. Chemical Safety and Hazard Investigation Board studied this incident and reported several issues-notable was that no hazard analysis had been performed. The case study of this incident provides a vehicle with which to teach the principles of hazard analysis and risk minimization.
