Sa1633 COMPLEMENTARY AND ALTERNATIVE MEDICINE USE IN PEDIATRIC PAIN PREDOMINANT FUNCTIONAL GASTROINTESTINAL DISORDERS

Functional gastrointestinal disorders (FGIDs) are a group of chronic, non-inflammatory disorders that can affect children and adults.The functional abdominal pain disorders, as defined by the 2016 Rome IV criteria, include functional dyspepsia, irritable bowel syndrome, abdominal migraine, and functional abdominal pain.There has been recent increased use of complementary and alternative medicines (CAM) in treating these pain predominant FGIDs.The aim of this study is to describe the use of CAM in patients with pain predominant FGIDs at a large midwestern pediatric GI center.Methods: We conducted a survey of returning patients to our clinic.Any patient up to 18 years of age attending a follow-up clinic visit with a diagnosed pain-predominant FGID (Rome III criteria) was considered.Patients with comitant inflammatory bowel disease were excluded as a large number have previously participated in a similar study at our institution.Demographics including age, sex, race/ethnicity, highest parental education level, family income, and type of insurance were obtained.Duration of diagnosis and perceived quality of life were also obtained.Families provided information on previous use of allopathic therapies including prokinetics, antacids, anti-spasmodics, antidepressants, laxatives, colonic secretagogues, and other allopathic medicines commonly used to treat FGIDs.Additional items pertaining to general health, opinions on CAM, and perception of their provider's knowledge of CAM were included as well.Broad categories of CAM included on the instrument included special exercise, vitamin and dietary supplements, dietary changes, stress management, and alternative professional care.Results: A total of 100 surveys were completed.47 respondents (60% female) had IBS, 29 respondents (83% female) had functional dyspepsia, 18 (67% female), and 6 (83% female) had abdominal migraine.Respondents were an average of 13.3 +/-3.5 years of age (range 4-18 years).Respondents had been diagnosed an average of 15 months prior to survey participation (range 1-120 months).The vast majority of respondents were white (88%).77% of all respondents described their quality of life as excellent or very good.Running was the most common form of exercised used by 34% of all respondents.Multivitamins and probiotics were the most common supplements used by 48% and 33% of respondents, respectively.Dietary changes were used by 61% of respondents with 20% avoiding multiple foods.Onequarter of respondents had seen a psychologist for care related to their FGID.Further associative and correlative data are provided.Conclusions: The use of CAM in children with pain predominant FGIDs is common with a majority of respondents experiencing a high quality of life despite their symptoms.
