Abstract
目的 总结开放性楔形胫骨高位截骨术(open wedge high tibial osteotomy,OWHTO)术中并发外侧铰链断裂的相关研究成果.方法 查阅近年国内外OWHTO相关文献,结合临床经验进行总结分析.结果 OWHTO术中发生外侧铰链断裂后,导致术后矫正角度丢失、截骨延迟愈合或不愈合等问题.外侧铰链对截骨术后稳定性起重要作用,术中可以采用“安全区”内截骨保护措施;当发生外侧铰链断裂时可选择TomoFix锁定钢板固定,以提供足够稳定性.对于发生外侧铰链断裂患者,可根据铰链断裂分型指导功能锻炼及确定完全负重时间.结论 完整外侧铰链利于OWHTO术后愈合及康复,OWHTO术中医师应高度重视,避免发生外侧铰链断裂.
