•Malignant lymphoma produces a bulky and hard tumor in the uterine cervix.•Only two cases of spindle cell variant DLBCL in uterine cervix have been reported.•R-CHOP chemotherapy is very effective for DLBCL.•Accurate diagnosis is essential to avoid unnecessary surgery or medical treatment.
