Abstract
The objectives of the study were to analyze the authors' experience in management of patients with postnecrotic pancreatic fluid collections (FC) and prove the surgical approach. 125 patients with FC were classified into two groups. Group 1 (n=73) included cases followed up for six months after the development of necrotizing pancreatitis (NP), group 2 (n=52)-more than six months after NP. FC external drainage, pancreas resection or resection combined with pancreatojejunal anastomosis were performed. The necessity for surgical treatment emerged in 54.8% of cases in group 1. The drainage under ultrasound guidance was the final stage of the operation in 45.8% of patients. There were 27 one-stage operations in group 2. 15 patients with complications underwent two-stage operations. Surgical treatment in acute FC is needed in half of the cases. Complicated postnecrotic cysts require staged operations.
