•The I-Fabulous study represents the largest cohort on I-FABP related to abdominal pressure and complications in ICU patients.•A relevant diagnostic value of I-FABP levels for identifying individual patients at risk for intra-abdominal pressure related complications could not be demonstrated•Our study does not support the routine use of I-FABP levels to help early identification of intra-abdominal pressure related complications in patients with IAH.
