IMPACT OF LIVER TRANSPLANTATION ON PATIENTS' HEMODYNAMICS AND RENAL FUNCTION

Abstract
Introduction: There are many risk factors for renal dysfunction after liver transplantation. We design this research to investigate the impact of liver transplantation on patients' hemodynamics and renal function and to analyze the possible causes so as to provide reference for clinical treatment and management. Case presentation: A total of 223 patients (with end-stage liver disease, American Society of Anesthesiologists (ASA) grade as Grade Ill or IV, and underwent liver transplantation in our hospital from March 2008 to December 2010) were selected as the study subjects to observe the changes of hemodynamics renalfitnction at different time points (after anesthesia induction (T1), 5-min anhepatic phase (T2), 20-min anhepatic phase (T3), 5 min after liver transplantation (T4), and 60 min after liver transplantation (T5). Conclusions: All the indexes at T2 showed significant decrease than those at T1 (P<0.05); central venous pressure (CVP), pulmonary artery wedge pressure (PAWP), mean pulmonary arterial pressure (MPAP), and heart rate (HR) at T3 showed significant decrease than those at T1 (P<0.05); systolic blood pressure (SBP) and diastolic blood pressure (DBP) at T4 showed significant decrease than those at T1 (P<0.05); while all the indexes at T5 returned to the levels at T1 . The postoperative content of Cr in the patients was significantly increased while the content of (CCr) was significantly decreased (P<0.05). According to our research, Liver transplantation has significant effects on hemodynamics and renal function in the patients, so such patients should be strengthened monitoring so as to prevent adverse reactions and sequelae.
