•Chronic azithromycin (AZM) is used for treatment of pulmonary disease in CF.•Macrolide use is associated with no risk to a small increased risk of congenital malformations.•AZM has a prolonged half-life in white blood cells.•Risk to the mother of AZM discontinuation must be weighed against potential risk to the fetus.
