Drug resistance in HIV-infected children living in rural South Africa: Implications of an antiretroviral therapy initiated during the first year of life.

•Long-term cART is effective in children independently of age at treatment initiation.•Majority of children present undetectable viral loads after 7 years of treatment.•Drug resistance mutations are more frequently detected in viruses from children treated early.•Better access to HIV genotyping in resource-limited settings is essential.
