•Angiotensin-II type I receptor antibody (AT1R-Ab) levels increased after left ventricular assist device implantation and remained until heart transplantation.•Increased AT1R-Ab level at time of heart transplant was associated with post-transplant morbidity.•AT1R-Ab levels did not correlate with surrogate markers of systemic inflammation.•Measuring AT1R-Ab levels during left ventricular assist device support could be a useful biomarker to identify LVAD patients at greater risk for poor outcomes after heart transplantation.
