•About 30% of treatment-seeking refugees with PTSD in a psychiatric outpatient clinic in denmark have secondary psychotic symptoms (PTSD-SP).•Depression is a common co-morbid condition that seems to influence the occurrence of psychotic symptoms in PTSD.•Among those with PTSD-SP, 44% also fulfilled the criteria for Enduring Personality Change After Catastrophic Experience (EPCACE).•Prevalence of psychotic symptoms was high, irrespective of the nature of the traumatic event. However, content of the psychotic symptoms was highly trauma related.
