•PFAS contamination of water detected in 2013 in an area of Veneto Region.•Severe small for gestational age newborn (SGA) studied 2003 to 2018 by the Veneto Birth Registry.•Large number of observations and covariates.•Findings support that PFAS in drinking water are adversely associated with SGA.•Individual data on exposure are needed to confirm this observation.
