Abstract Background The Z-shaped Elongating Varisating Ostetomy (ZEVO) is presented and described technically. Methods Its purpose is to be part of a joint preserving reconstruction for acquired flatfoot deformity (AFFD) in adults. This is possible in a still flexible deformity. The concept evolved from the double ostetomy and by connecting these with a horizontal cut. The need for grafting is avoided and a linear lenghtening of the calcaneus is achieved. Results and Conclusions The technique has been in use for more than 20 years in AFFD reconstruction. No lateral sided symptoms were observed as with other calcaneal lengthening procedures. The ZEVO appears to be more powerful and corrective than an isolated medial translation of the tuberosityly or lateral lenghtening.
