•Appraise the current literature on the parental decision-making experience regarding medical care of a child with a prenatally identified life-limiting diagnosis.•Apply the palliative care skill set used in exploration of goals of care during the prenatal period.•Design a perinatal palliative care birth plan that will enhance communication of value-driven goals of care and function as an advance care planning tool.•Discuss the role of perinatal palliative care-supported advance care planning in enhancing opportunities to parent at the end of life. Advances in prenatal care and diagnostic technologies have allowed for an increase in the numbers of families seeking support for decision-making in the context of delivery and care of a newborn with a prenatally identified life-limiting diagnosis. Perinatal palliative care (PPC) consultation as a part of interdisciplinary fetal care offers opportunities to explore values and preferences surrounding goals of care (GOC) as families plan for the delivery and medical care of their newborn. Creation of a perinatal advance care plan enhances parental autonomy and communication with care teams. It gives the family a stronger voice in preparing for their child's life and can aid in expanding perceived options for newborn care. While GOC for many families ultimately align with comfort-focused medical care, requests for subspecialty evaluation and the possibility of life-sustaining treatments and interventions are not uncommon. Exploration of GOC, assistance in framing limitations of intervention, and advocacy for sound symptom management are best supported by palliative care-trained providers. In this concurrent session, PPC providers will illustrate how creation of the palliative care birth plan can function as an advance care planning tool. We will utilize case examples and a review of literature to provide a broad perspective on the parental decision-making experience regarding the medical care of a child with a prenatally identified life-limiting diagnosis. We will examine the palliative care skill set used in exploration of GOC during the prenatal period. Using our multi-institutional experiences, we will describe creation of a PPC birth plan and its role in enhancing communication of value-driven GOC. Finally, we will employ case examples and our collective professional experience to describe the role of PPC-supported birth-planning in enhancing opportunities to parent at the end of life, create memories, and build a legacy for the newborn with a life-limiting diagnosis.
