Laparoscopic partial duodenectomy for the non-ampullary tumors of the duodenum

Abstract
Introduction: Duodenum tumors locating in the non-ampullary region remained to be a clinical challenge. Traditionally, these lesions require en bloc excision of the periampullary region performed in an open surgery fashion. The aim of the present study was to evaluate the clinical efficacy and safty of the laparoscopic partial duodenectomy followed by side-to-side duodenojejunostomy. Method: The clinical data of 14 patients with non-ampullary lesions who underwent laparoscopic partial duodenectomy followed by side-to-side duodenojejunostomy in the Department Of General Surgery, Peking University Third Hospital was collected and analyzed. Result: The median duration of operation was 194 min (range: 140-530 min), and median estimated median blood loss was 35 ml (range: 10-700 ml). Two patient was converted to open surgery. The median postoperative hospital stay was 10 days (range 6-35 days). No postoperative mortalities occurred. Only 3 patients were febrile on postoperative day 1-7. Postoperative complications included delayed gastric emptying (DGE) (n=2), duodenojejunostomy stricture (n=1), and intestinal obstructions (n=2). Conclusion: Laparoscopic partial duodenectomy gained safety and feasibility for treatment of duodenal lesions not involving the ampulla.
