•Intensive induction chemotherapy met response targets in poor prognosis germ cell tumours.•Three-year progression-free survival was 56% for CBOP/BEP and 39% for BEP.•Impact on survival was less clear and will be affected by subsequent therapy.•Further study in an international phase III trial is warranted.
