•Countrywide programmatic roll-out shows successful MDR-TB management in Niger.•The short treatment regimen resulted in 83·1% % relapse-free success.•Successful outcome was not affected by initial resistance to companion drugs.•Few patients with relapse or failure cured with bedaquiline-based regimens.•Audiometry to switch from injectable to linezolid prevented severe hearing loss.
