•We report patient outcomes from a monthly buprenorphine extended release (BUP-XR) trial.•Quality of life was stable or improved from BUP-XR start to 49 weeks (EOS).•Treatment Effectiveness Assessment scores improved from BUP-XR start to EOS.•Employment rate increased by 7% from BUP-XR start to EOS.•Over 88% of patients reported medication satisfaction at EOS.
