Discussing a Nurse Assisted eHealth Service From Hospital to Home

Abstract
Non-communicable diseases are prone to comorbidity and complex health care needs, with the risk of experiencing burden of treatment. Current health care practice on post-discharge follow-up of patients with noncommunicable disease is improper and the knowledge about how to mend it is insufficient. The aim of this paper is to present and discuss an innovative Nurse Assisted eHealth Service from hospital to home for people with noncommunicable diseases of heart failure and colorectal cancer. We present a model of a remote eHealth service, and by drawing on earlier research and Burden of Treatment theory, we discuss the eHealth service potential to optimize patient experience, decrease burden of treatment and increase health-related quality of life.
