•Patients with biopsy Gleason score 8 (4 + 4) have heterogeneous pathologic outcomes.•Over half of Gleason score 8(4 + 4) cancers are downgraded at radical prostatectomy.•Low volumes of pattern 4/presence of pattern 3 are associated with downgrading.•Downgraded patients have improved oncological outcomes.
