•Obstructive sleep apnea syndrome (OSAS) prevalence in childhood stroke is high.•OSAS correlates significantly with poor neuropsychological function.•Our study highlights the importance of screening for OSAS in children with stroke.•Future studies should investigate therapeutic interventions for OSAS in stroke.
