•Rapid changes in lung cancer treatment can cause budgetary issues.•The iTEN model was built to assess the impact of new lung cancer therapies.•The model validated well to five global real-world data sets.•Canadian treatment of advanced non-small cell lung cancer (aNSCLC) was modelled.•aNSCLC patient 5-year survival was an estimated 3% and cost $110,806 per patient.
