Abstract
Background Immediate implant placement following tooth extraction in the esthetic zone is a treatment who has been described since many decades. In the beginning the only question was if osseointegration really occurs. However, soon became evident that the immediate implant placement involves certain risks, especially when it is combined with immediate temporization. The IDR technique constitutes a treatment that surpasses most of these risk factors and provides a superb outcome. Aim/Hypothesis Presentation of the benefits and the restrictions of the IDR technique along with a case presentation in order to discuss the treatment process with all the stages involved. Material and Methods Case presentation of a female patient in which the concept of the IDR technique was used to restore a non-restorable 21. First atraumatic extraction was done using luxators, after the area was observed showing buccal bone dehiscence. Tuberosity bone was harvested with chisels and it was trimmed in order to fit and replace the buccal bone. The implant was placed in order to have a screw-retained prosthesis and the gap between the implant and the buccal bone was filled with bone particles from the tuberosity. After 4 months clinical and radiographic examination was done showing excellent results. A final impression was taken and a screw-retained metal ceramic crown was fabricated. Results Immediate implant placement followed by temporization of the implant in the esthetic zone constitutes a treatment modality that involves many risk factors and does not always provide an excellent esthetic result. However, the IDR concept eliminates most of these risk factors with great success and outstanding esthetic results. Conclusion and Clinical Implications The use of the IDR concept allows the immediate implant placement and the immediate temporization of the implant providing great clinical and radiographic results. It, also, eliminates the need for many surgeries reducing patient's discomfort and increasing treatment acceptance. Moreover, it has a positive effect on the financial aspects of implant treatment.
