•Providing thrombectomy services to rural or remote regions with small, dispersed populations often requires aeromedical retrieval.•The logistical challenges of aeromedical retrieval for rural and remote large vessel occlusion stoke patients are poorly studied.•The state thrombectomy unit of Western Australian has a vast rural catchment of over 2,500,000 km2.•This review of the state thrombectomy registry demonstrates that even when large distances are involved, rural large vessel occlusion stroke patients can have excellent outcomes after being aeromedically retrieved for thrombectomy at a high volume centre.
