•Prediction of the proximal extent of thrombus formation in PAO is difficult.•Hemodynamics after PAO for a cerebral aneurysm was simulated with CFD.•Specific wall shear stress and shear rate thresholds were used for the simulation.•Preoperative CFD was consistent with the postoperative DSA.•CFD was useful in predicting the extent of thrombus formation in the parent artery.
