1-About 25% of patients suffering from end stage heart failure (ESHF) are obese.2-BMI > 40 prevents patients from receiving an organ because morbid obesity represents a contraindication to registration on the transplant list.3-Successful management of an obese patient suffering end stage heart failure (ESHF) by the combination mechanical circulatory support using a Left ventricular assit device LVAD and bariatric surgery using laparoscopic sleeve gastrectomy(LSG).4-Setting: Lausanne University Hospital CHUV, Switzerland.5-Video case report showing LVAD followed 10 months later by Laparoscopic sleeve gastrectomy in a 54-year old obese patient.6-Patient lost 22 Kg in the first 3 months after SG (BMI<35), Excess Weight loss (EWL= 45 %) he is now in the heart transplant list7-The LVAD driveline was visualized but did not interfere with the bariatric surgery procedure. No intraoperative or postoperative complications were observed.8-Bariatric surgery represents an effective safe therapeutic strategy to make ESHF obese patients eligible for heart transplantation.
