Abstract
run-off, at which time absorbable gelatin sponge was injected as a slurry for hemostatic reinforcement.Results: 10 consecutive patients underwent EUS guided coil embolization reinforced by absorbable gelatin sponge.Table 1 summarizes baseline patient characteristics; Notably, 90% (9/10) had actively bleeding or recently bled GV.Mean Child Pugh score was 8 [Class B] with mean MELD of 14.Table 2 summarizes procedural results.Technical success, defined as uncomplicated injection of coils and sponge was achieved in 100% (10/ 10).A mean of 8 coils were deployed per case with an average total coil length of 119 cm.A mean of 3.4 cc of absorbable sponge was injected.In one case, absorbable sponge was not injected due to persistent contrast run-off.All patients had immediate cessation of transfusion requirements.Mean follow-up was 6 months [5-397 days]; No patients re-bled or required blood transfusions or required re-intervention for bleeding.Complication rate was 10% (1/10; severe abdominal pain without concerning radiographic findings); otherwise, there were no cases of systemic embolization.One patient passed away from non-GV related causes.80% (8/10) had follow up EUS [mean 3 months]; 100% (8/8) revealed nearobliteration of GV with significant reduction of Doppler flow.Discussion: EUS-guided coil embolization in combination with absorbable hemostatic gelatin sponge is a novel method for the management of bleeding GV and is associated with high technical and clinical success rates.Unlike cyanoacrylate, absorbable gelatin sponge used as a hemostatic adjunct appears to have a favorable safety profile.This combination technique minimizes embolic complications and reduces the need for re-intervention.Further studies are required to compare this modality to alternative treatments available for GV.
