摘要
e21533 Sarcomas of the digestive tract (excluding GISTs) (SDT): a study of 63 patients from the French Sarcoma Group (GSF) database Background: The natural history of SDT has been rarely considered. We describe the histoclinical  characteristics of SDT from the GSF database and analyze prognostic factors. Methods: We performed univariate with Fischer test and multivariate analysis with Cox regression for Overall Survival (OS), Metastasis-Free Survival (MFS), Local Recurrence-Free Survival (LRFS) and Disease free survival (DFS). Results: Twenty-one out of 84 patients (25%) who were metastatic at diagnosis were excluded. We restricted our analysis to patients with local disease to obtain a more homogeneous population (N=63). SDT represent about 0.8% of all soft tissue sarcomas. The median age was 60 years (range 18-83). Tumor locations were small intestine 34.9%, colon 23.8 %, stomach 19%, rectum 12.3%, oesophagus 4.8%, duodenum 4.8 %. Median tumor size was 8.5 cm (range 0,3-30). The most frequent tumor types were leiomyosarcoma (44.5%) and unclassified sarcoma (23.8%). 48.1% of cases were grade 3. 90.3% of patients underwent first surgery : 27.4% were treated in a GSF referral center. 75.5% patients had R0 resection. Surgery was followed by radiotherapy for 11.9% patients. Adjuvant or/and neoadjuvant chemotherapy was given only to 9 (15,0%) patients. The 5-year OS, MFS, LRFS and DFS rates were 60.1%, 74.1%, 78.2% and 54.5% respectively. Multivariate analysis retained unclassified sarcoma histological type for predicting OS (p = 0.036; OR = 3.33). Surgery in referral center appears as a factor of good prognosis in terms of DFS in univariate analysis (p = 0.048). Conclusions: Unclassified SDT had a significantly poorer OS compared with others histotypes of sarcoma. The 5-years MFS and LRFS rates appeared similar compared to sarcomas of other locations. This retrospective study underscores the importance of referral care center with a multidisciplinary sarcoma team.  Â
