Can We Predict Long-Term Mortality Post Primary Takotsubo Syndrome Via In-Hospital Events?

Background: Some patients with Takotsubo syndrome (TS) develop this form of acute catecholamine-induced myocardial inflammation secondary to life-threatening extra-cardiac disease (TSsec), but in the majority of cases, TS occurs in isolation (“primary” TS [TSpri]). Considering all cases, TS carries a substantial long-term mortality, but outcomes following TSpri and their determinants have not previously been investigated. Purpose: We sought to determine long-term mortality rates for TSpri patients, and to identify early predictors of mortality risk. Methods: Consecutive TSpri patients (n = 235) were followed up for a median duration of 3.3 years [interquartile range 3.1-6.2 years]. Associations between demographics, severity of index attacks (measured via haemodynamic impact, inflammatory and necrotic markers, and extent of catecholamine release) and rate of major adverse events were sought utilising univariate analyses followed by multiple logistic regression analysis.
