Abstract
Newly developed life-prolonging drugs (LPD) for castration resistant prostate cancer (CRPC) are palliative with similar survival benefits compared to standard of care. Patient reported outcomes are therefore important. The purpose of this study is to investigate the cancer-specific quality of life (QoL) in CRPC patients. PRO-CAPRI is an observational, prospective study in 10 hospitals in the Netherlands. CRPC patients from diagnosis to start of first post-docetaxel treatment were eligible. EORTC QLQ-C30 was answered three-monthly for a maximum of 2 years. EORTC QLQ-C30 consists of functional and symptom scales, ranging from 0 to 100. High scores on functional and symptom scales represent high level of functioning or high level of problems respectively. Clinical relevant deterioration was defined as 10-point difference from baseline. 151 CRPC patients answered a median of 6 questionnaires per patient. Median follow-up was 19.5 months. 59% terminated the study early, due to death (31%), lost to follow-up (19%) or withdrawal of consent (9%). 91% was treated with LPD prior to or during the study. At study start, highest functioning was measured on social (median 100, IQR 66.7-100) and emotional functioning (median 91.7, IQR 50-100). Most reported symptoms were fatigue (median 33.3, IQR 11-44) and pain (median 16.7, IQR 0-33). During follow-up, a certain degree of deterioration was seen on almost all functional scales, except on emotional functioning. Clinically relevant deterioration was measured in role (ability to perform work, daily or, leisure time activities) and physical functioning. The symptoms appetite loss and fatigue also showed relevant deterioration. Although new treatments are available, CRPC has an impact on cancer-specific QoL with deterioration in almost all functional and symptom scales. Specific domains of relevant deterioration as role functioning and physical complaints (appetite loss and fatigue) have been identified and need special attention during the follow-up of CRPC.
