Ist Wahn indirekt veränderbar? Ein stimmungsverbesserndes Konzept der kognitive Verhaltenstherapie für die stationäre Standardversorgung von Patienten mit schizophrenen Störungen

Background: The efficacy of cognitive-behavioral therapy for psychosis (CBT-p) has been proven in the treatment of schizophrenic disorders. However, CBT-p concepts have not yet been implemented completely in in-and outpatient primary care. The aim of the present study was to assess the feasibility and potential effectiveness of a mood-stabilizing CBT-p concept for an inpatient clinic based on recent research findings on the role of depression in the formation of delusional symptoms. Patients and Methods: 16 patients of a ward for schizophrenia spectrum disorders received a manualized CBT-p for mood stabilization in an individual setting. Sociodemographic characteristics, delusional symptoms, general psychotic symptoms, depression, and chlorpromazine equivalent doses were measured at baseline and at a post-assessment and analyzed with a repeated-measures design. Results: Delusional symptoms were reduced significantly from baseline to post-treatment with large effect sizes in both self-report and observer-rated and blinded measures (effect sizes between 0.69 and 1.19). Depressive symptoms also decreased significantly with a large effect size (1.21). Conclusions: The findings of the present study argue for the effectiveness and feasibility of a mood-stabilizing CBT-p concept. These results should be replicated in a randomized controlled trial to consolidate the evidence and improve CBT-p interventions in terms of practical applicability and effectiveness (C) 2018 S.
