•Study region is Brazil, with higher endemic of HBV.•Screening consisted of >12,000 HBsAg tests in 7 years (7,763 pregnant women).•Medical records of 75% newborn of HBsAg+ mothers were without prophylaxis records.•Only 14% of children of HBsAg-positive mothers had follow-up postimmunization.
