•HHV-8 DNA levels in plasma samples are not useful in assigning PEL status.•Effusion fluid HHV-8 DNA copy to cell ratio provides a useful predictor for PEL.•A ratio of <1 copy to cell almost certainly excludes PEL.•A ratio of>100 almost certainly indicates PEL.•A ratio of >1 < 100 falls into a “grey area” relating to PEL.
