•Upfront transplantation in high-risk MDS and sAML is at least not inferior compared with pretransplant cytoreductive approaches.•Pretransplant treatment may impact response and survival after salvage therapy in patients who relapse after allo-HSCT.•An upfront transplant strategy may be augmented by HMA + donor lymphocyte infusion salvage therapy in case of relapse after allo-HSCT.
