P167 Understanding the mechanisms for the increased prevalence of gastro-oesophageal reflux in cystic fibrosis

Abstract
Methods: Data has been collected prospectively on all patients receiving gliptins from March 2017 onwards.Demographics and biomedical parameters (FEV1, BMI, HbA1c) and CGM data (% time blood glucose >8 mmol/l, % time in target, % time <4 mmol/) were collected prior to initiation of a gliptin and at follow up (2-6 months).Results: 22 patients were identified as having significant glycaemic excursions and were commenced on a gliptin (9.7% of our adult cohort).At present, follow up HbA1c data is available for 10 patients and CGM data for 5: by May 2018 this should be nearer 100%.Median HbA1c fell from 44 mmol/mol to 40 mmol/mol ( p = 0.04) and the median time in the target blood glucose range increased from 67% to 85% ( p = 0.05) from pre to post starting a gliptin.There was no significant difference found between BMI or FEV1.Conclusion: These preliminary data indicate clinically relevant improvements in glucose homeostasis and support the use of gliptins, particularly for patients with moderate glucose excursions, barriers to insulin therapy or with poor adherence.Longer term follow-up data will be required to determine the impact of this intervention on overall CF outcomes.
