•Alcoholism is linked to sexually dimorphic changes in the medial forebrain bundle.•Alcoholic men had lower FA and higher RD compared to normal men controls.•Alcoholic women had higher FA and lower RD compared to normal women controls.•Higher number of daily drinks was associated with greater brain damage in women.•Findings suggest sex-specific vulnerability to alcohol-related brain damage.
