Introduction: There is care fragmentation and increased diabetes-related hospitalizations during transition to adult type 1 diabetes (T1DM) care. We aimed to understand attendance patterns at a Young Adult (age 18–25) T1DM program and used the Model for Improvement framework to reduce non-attendance and improve care delivery.
