EFFICACY OF THE RONNIE GARDNER METHOD: PILOT RANDOMIZED CONTROLLED TRIAL IN OLDER ADULTS WITH MILD COGNITIVE IMPAIRMENT

Abstract
The number and proportion of older adults with mild cognitive impairment (MCI) is growing. Novel interventions that jointly deliver cognitive and physical challenge may improve cognitive function. The Ronnie Gardner Method (RGM) delivers activity following a symbol based script, a new language, and progressive challenge in timing and activity type, all set to music. We examined the feasibility of a randomized controlled trial of the RGM, and established potential efficacy at improving cognitive and physical function in older adults with MCI. Thirty older adults (mean age 80.5 years) with MCI, living independently, were randomized into either the RGM, free-movement dance, or seated music appreciation groups, for 60 minutes weekly for 12 weeks. The primary outcomes were feasibility and acceptability; secondary outcomes were cognition (CNS-Vital Signs) and physical function (Short Physical Performance Battery) compared between the group using nonparametric analyses and intention-to-treat principles. Twenty-six participants completed baseline and follow up assessments (effectiveness analysis), and 6 participants in each arm completed ≥ 6 sessions (efficacy analysis). The RGM group reported significantly greater agreement that activity helped their overall health and their memory; and a greater willingness to engage in the activity again than the free-movement dance and seated music appreciation groups. The cognitive scores non-significantly improved in the RGM efficacy analysis (p=0.06) and significantly improved (p=0.02) in the physical function scores. No adverse events to report. The RGM is safe, acceptable, and feasible for older adults with MCI, improves physical function and may protect against cognitive decline. A definitive trial to test the RGM for cognition is feasible.
