•We assess the economic impacts of illness with health but no earnings insurance.•Substantial economic illness risks remain under the Thai Universal Coverage Scheme.•Severe illnesses increase out-of-pocket medical spending and cause earnings loss.•Spending on non-medical goods is protected by informal transfers, credit and savings.
