Abstract P-553: FIRST PEDIATRIC PATIENT WITH 50 ML VOLUME TAH IN TURKEY

Aims & Objectives: Total artificial heart (TAH) is a kind of mechanical circulatory support. Using TAH as an interim solution before the heart transplant in bi-ventricular dysfunction has been a common practice recently. Methods Employing mechanical circulatory support like LVAD and TAH in later stage heart dysfunctions due to dilate cardiomyopathy is becoming more common. LVAD already has a widespread use as an interim treatment option. But TAH may be better suited for patients with bi-ventricular heart dysfunction or for those patients who have structural impediments. We are presenting the first patient with a 50 mL TAH in Turkey. Results 15 years old, no previous known disease, 1.45 m2 male patient, started presenting heart dysfunction three months ago. He has diagnosed with “LV non-compaction dilated cardiomyopathy”. Catheterization revealed PA 37/27/32 mmHg, RV 40/18/17 mmHg, Aort 74/40/44 mmHg, RA 24 mmHg, PCWP27 mmHg, TPG 5, and RVSWI 146. He has intubated and need continuous renal replacement therapy in the PICU. Due to the continuation of circulatory dysfunction even with the high dosage of inotrope support, he was connected to VA-ECMO. Then it was decided to implant TAH to bridge him to heart transplantation. In the 8th day of VA-ECMO, TAH (SynCardia Systems Inc. Tucson USA) was implanted to the patient. He was discharge after 2 months implantation with favorable neurologic outcome except hearing loss. Conclusions TAH is a lifesaving method that can be applied as a bridge treatment until total heart transplant in the patients with advanced biventricular heart dysfunction.
