•It is possible to prevent progression to type 2 diabetes in high risk individuals.•Risk of complications from type 2 diabetes can be reduced by good early control.•The Berlin Declaration proposes simple policies to target these issues.•It is hoped that this will empower people to take action and improve outcomes.•Primary care physicians have a key role in implementing the Berlin Declaration.
