We sought to determine if stone formers who had surgery differ in stone related quality of life (SR-QOL) compared to those who have not had surgery. METHODS After IRB approval, adult stone formers at 6 centers completed a WISQOL questionnaire; medical and surgical data were compiled. We divided patients into 2 groups: those who had kidney stone surgery within 1 year (surgery) and those that never had stone surgery (no surgery). We calculated WISQOL total and domain scores, standardized to 0-100 (domains: D1 social functioning; D2 emotional functioning; D3 stone-related impact; and D4 vitality). For the surgery group, we identified patients that had shockwave lithotripsy (SWL), ureteroscopy (URS) or percutaneous nephrolithotomy (PCNL) within 0-3.0 months or within 3.1-12 months. Scores between and among groups were compared. RESULTS Except for higher BMI in the surgery group (32.1 ± 9.2 vs. 28.9 ± 6.9 kg/m2, p<0.001), demographics did not differ. Within 3 months of surgery (n=103; 71 URS, 15 SWL, 15 PCNL) SR-QOL was significantly lower (60.3 ± 25.7) than for no surgery (n=175) (74.6 ± 23.7) (p<0.001), specifically for those who underwent SWL (58.4 ± 24.1) and URS (58.6 ± 25.8) but not for those who underwent PCNL (67.9 ± 26.5). The only domain score that differed by surgery type within 0-3.0 months was D3. In this time frame, patients who had SWL (47.3 ± 29.3) scored lower than those who had PCNL (69.8 ± 27.7) (p=0.02). For each WISQOL item, the percent difference in score between patiens who had surgery within 0-3 months and those who did not is shown (figure). For 3.1-12 months from surgery (n=95; 69 URS, 12 SWL, 14 PCNL), SR-QOL was higher (81.1 ± 74.7) compared to no surgery (74.6 ± 23.7) but this was not statistically significant (p=0.122). The only domain score that differed in this time frame was D1 for URS (89.5 ± 19.1) compared to no surgery (81.1 ± 26.2) (p=0.016). CONCLUSIONS SWL and URS were associated with lower SR-QOL within 3 months of surgery, while PCNL was not in this cohort. Post-operative SR-QOL within 1 year is comparable to those with no surgical stone history.
