FoundationOne (Foundation Medicine, Cambridge MA) is a validated comprehensive genomic profile (CGP) for solid tumors designed to provide clinically actionable information to guide treatment decisions based on a tumor′s genomic profile. Our institution is a member of the Precision Medicine Exchange Consortium (a collaboration between 8 academic cancers to facilitate data exchange, advance research, and support education and applications of precision medicine in oncology and molecular pathology with Foundation Medicine. We reviewed our initial experience with CGP in a series of patients with genitourinary malignancies, most of whom had failed standard therapies. METHODS A retrospective review of our patients with genitourinary cancers sent to Foundation Medicine for CGP between 2/1/2013 and 10/31/2017. RESULTS A total of 150 samples were evaluated by CGP (table 1). At least one gene alteration was identified in 145 (97.0%) of the cases with a total of 185 unique mutations among the cohort. Each patient had a mean of 4.8 (SD 3.0) mutations (range 1-16). For all patients, a total of 45 (31.0%) mutations were identified with FDA approved agents for that disease. 88 (61.0%) patients had mutations with an available therapy FDA approved for another disease. There was at least one clinical trial available to 124 (86.0%) patients. 21 patients (14.0%) had an identifiable genetic alteration and no clinical trial available. Table 1 shows the breakdown of mutations for typical histologies of prostate, urothelial, and clear cell kidney cancer. CONCLUSIONS Tumor genomic profiling using platforms such FoundationOne can yield useful information about specific mutations and druggable targets for patients progressing on standard therapies. This information can lead to informed decision making for determining treatment sequence and also encourage clinical trial participation.
