Abstract
e14569 Background: Stereotactic body radiation therapy ( SBRT) has been used for oligometastatic disease management with local control rates in excess of 90%. Oligoprogression is the term applied to limited disease progression, with overall disease control. Standard care would be changing systemic therapy. A novel approach is to attempt to locally control the progressing lesions and continue the same systemic therapy. For patients on a Tyrosine kinase inhibitor (TKI), progressing lesions were found to accumulate new mutations that cause resistance to the TKI. Hence, treatment with non-overlapping mechanism of resistance such as radiation may be offered. This approach was reported in a retrospective study of patients with anaplastic lymphoma kinase (ALK) positive metastatic non-small-cell lung cancer treated with crizotinib, who received SBRT to oligoprogressive disease.
