•In children, after allogeneic hematopoietic cell transplantation, more than 1 episode of blood stream infection was associated with a significant increase in the risk of resistance to piperacillin-tazobactam, cefepime, and vancomycin•An infection with an antibiotic-resistant organism was not associated with transplantation-related mortality•Development of enteric bacterial blood stream infection after the onset of acute gastrointestinal graft-versus-host disease was the strongest predictor of transplantation-related mortality
